Nancy Bocanegra with Community Health Choice is providing information and answering questions about CHIP, a health insurance program for Texas children, at May Community Center Thursday, February 18 from 1:30 p.m. to 4 p.m. Plan to attend this informative program to learn more about the qualifications, coverage, and costs of CHIP.

CHIP covers children (birth through age 18) who do not qualify for Children’s Medicaid. Some of the services covered by CHIP include doctor’s office visits, vaccinations, eye exams and glasses, dental care, lab tests and x-rays, home healthcare, and much more. To apply, please bring the following documentation to this program:

  • Parent or legal guardian’s social security number (if you do not have a social security number or are not a U.S. citizen, you may still qualify for the CHIP Perinatal program)
  • Child’s social security number and birth certificate or proof of legal residency status
  • Copies of paycheck stubs for the past 60 days, or a copy of last year’s tax return
  • A dated and signed letter from employer verifying cash income (proof must be submitted for each working family member)
  • Child support receipt
  • Most recent social security statement
